City of Granite Falls Passport and Photo ServicesPlease have the following ready when you come in:
Check or Money Order for payment.
Completed DS-11 application form (for each applicant).
If you are planning for international travel, please check the country information on the U.S. Department of State’s website (click here) to see if there are any passport validity requirements for the country you wish to visit.
Processing times begin the day the Department of State receives your application at a passport agency or center, not when you mail your application or apply at a local acceptance facility.
Suppose you have urgent or emergency travel requirements or plan to travel within five to seven weeks. In that case, you will need to contact the National Passport Information Center at 1-877-487-2778 to schedule an appointment with the Seattle Passport Agency. When you meet with a Passport Technician, please bring the following:
Payment for the passport fee must be either a check or money order payable to the U.S. Department of State (No Counter Checks).
The processing fee of $35.00 for each application must be cash, credit/debit card, check, or money order payable to the City of Granite Falls. If using a credit/debit card, a processing fee may apply.
Copies of the front & back of your driver’s license for each application; if the license is less than one year old from the date of issuance, you will be asked to provide a second picture I.D. and additional photocopy of the front and backside of the document
Proof of U.S. citizenship. (a certified copy of a birth certificate, Naturalization Certificate, or previous passport) (additional photocopy of birth certificate or Naturalization Certificate is now required (front & back) single-sided on a full sheet of paper for each application)
Any name change documents (Court Documents or Marriage Licenses)
One passport photo which meets federal requirements (optional, we have photo services available for $20 per person) – No glasses allowed in photo.
Children must appear in person. If renewing a child’s passport (under age 16), you must submit the current passport and a certified copy of the birth certificate.
Completed DS-11 Form (in black ink) – available online, or hard copies are available at City Hall.
Citizenship Evidence – state certified birth certificate with B/W photocopy of front & back; or previous U.S. passport.
Passport Photo – taken in the last six months (optional, we have photo services available for $20 per person).
Proof of Parental Relationship – state-certified birth certificate.
Parents’ Valid Identification – Washington driver’s license with B/W photocopy of front & back (if applying with multiple minors, a photocopy of each I.D. for each application)
Completed DS-11 Form (in black ink) – available online, or hard copies are available at City Hall.
Citizenship Evidence – state certified birth certificate with B/W photocopy of front & back; or previous U.S. passport.
Valid Identification – Washington driver’s license with B/W photocopy of front & back (all applicants age 16 and older must have their photo I.D. of some kind).
Passport Photo – taken in the last six months (optional, we have photo services available for $20 per person).
Passport Photo – taken in the last six months (most drug stores and FedEx stores offer passport photo services).
Most Recent Passport – (issued no more than 15 years ago).
Payment – must be a check or money order payable to the U.S. Department of State.
We recommend the flat rate envelopes from the United States Post Office for mailing. You will receive a tracking number when you pay your postage to ensure your sensitive information arrives at the U.S. Department of State.
